Provides short-term (3 to 6 months) and longer-term (up to 1 year) case management support for people experiencing mental health and/or addictions concerns. Services focus on recovery planning, weekly counselling and community-based supports, including assistance with housing applications, service coordination, and goal setting. Referrals are completed through SHIP Central Intake and Access
