Description des services:
|
Provides housing and wraparound supports to adult men in the community overcoming situations of homelessness due to acute mental illness and/or substance use. Clients are required to demonstrate a reasonable level of stabilization in their recovery journey as well as a readiness to live a substance-free lifestyle
|

Frais:
|
Residential services are charged via social benefit guidelines and based upon a rent geared to income (RGI) model for those in the New Leaf and RRH programs.
|

Procédé et formulaires:
|
New Leaf Program: Admission to the program is gained through the completion of a written application Service provider and self-referral accepted. RRH Graduate Program: Internal SLPP referral applications only. Email referral/application to intake@slpp.ca.
