Description des services:
|
Provides support for people with chronic health conditions, including diabetes. All workshops are group-based and non-clinical
Peer-led group sessions include chronic disease self-management, diabetes self-management and chronic pain self-management. Each six-week session runs for two and a half hours, once a week
Topics include:
- setting personal goals
- creating realistic action plans
- identifying and solving common problems
- managing pain and fatigue
- developing healthy eating and physical activity plans
- managing depression, stress and difficult emotions
- managing medications
- working with health care providers
- planning for the future
Other workshops offered:
- Diabetes, healthy feet and you: Learn how to keep your feet healthy and prevent diabetic foot ulcers through good diabetic foot care practices
- Getting the most from your health care appointment: Learn what to do before, during and after a health care appointment to optimize your health
- Mindfulness awareness stabilization training: Mindfulness practices for people experiencing anxiety and depression
- Powerful tools for caregivers: Self-care tools and support for caregivers of people living with chronic conditions
- Help with your health: Learn how to make a plan that supports the health changes that are important to you
