Description des services:
|
Clinic for youth who:
- have been diagnosed with a mental health disorder and are at high risk of harming themselves or others, or
- are at risk of developing impairments due to a mental health diagnosis
TAY patients receive helps to:
- be involved in their own care
- learn new skills and coping strategies
- maintain healthy lifestyles
- return to school, work and/or daily life
- smoothly transition into adulthood
The clinic offers:
- virtual care via telephone or video call platforms (e.g. OTN, Zoom)
- medication education and management
- psychosocial assessment
- up to ten individual counselling sessions and/or group programming. Family members may also be invited to attend individual sessions
- support for transitions to adult mental health care, addictions services, educational and/or employment opportunities
- referral to community services
- flexible hours, including evening and weekend appointments
Services are offered for up to six months

Youth can request a referral to this program from their family physician or psychiatrist

Admissibilité / population desservie:
|
Youth aged 16-24 years * live in the Central West Region * have one of the following: a major mental illness ; new and emerging mental health concern ; suicidal threats and or ideation ; concurrent substance use
