Description des services: CHO provides shared accommodation and supportive programming for individuals living with serious mental illness who may benefit from a 24-hour supportive care environment within the community

The program is grounded in a person-centred and recovery-oriented approach, recognizing that each individual’s needs, strengths, and goals are unique and may change over time. Clients are supported as active participants in their own goal setting and care planning

The service approach includes:

  • community mental health counselling
  • nursing support
  • financial literacy counselling
  • coordination and brokerage to other health and community services
