Description des services:
|
Program for seniors experiencing serious and persistent mental health concerns which include: depression, mood changes, dementia, responsive behaviours, and/or anxiety
Seniors having challenges accessing supports due to geography, isolation or changes in informal support system are considered a priority for this service
- makes referrals to, and collaborates with, community partners such as Ontario Health atHome-Brampton (Central West), Seniors Day Programs, Alzheimer’s Society of Dufferin and local hospitals
- education and support to the client and care team, to ensure clients have the necessary services and supports to live successfully in the community and to maintain/improve quality of life
