Personal Support Services
Transitioning Services: transitioning assistance, hospital (discharge)/rehab to
home, moving/downsizing
Activity Services: mobility assistance and fall prevention, turning, transferring and positioning, feeding (special diet) / meal assistance / nutrition,assistance with toileting, bedside monitoring and supervision, walking and exercising
Support Services: alzheimer’s, parkinson's, dementia and end of life care; respite for caregivers, 24 hour care, light housekeeping and laundry, light meal preparation, companionship and conversation, grocery shopping, errands,event planning, medication reminders, prescription pick up, daily wakeup call and check in visit
Hygiene Services: hair and mouth care, grooming and shaving, skin care prevention), incontinence care, dressing and undressing, bathing, washing, showering
